Transition Services

Initiating and maintaining collaborative relationships with targeted skilled nursing facilities. They develop innovative referral mechanisms based on skilled nursing facility internal operations, needs and regulations to facilitate guiding consumer referrals for transition services.  They also develop contact base of personnel at targeted institutions, and track related referrals to guide outreach efforts. They are responsible for conducting consumer transition assessments and complete ICAP standardized scoring to determine needs, and capacity for community living.  Analyze physician and rehabilitation reports for use in service planning and in establishing benefits eligibility.

Working with the consumer they develop individualized Transition Plans in conjunction with relevant external providers.  Oversee all aspects of service delivery on transitioning consumers in conjunction with other ILC staff until successful transition occurs.
